Nurse Practitioner – Primary Care (Mobile)

We are seeking a compassionate and motivated  Nurse Practitioner to join our mobile care team. This role is a unique opportunity to bring personalized, preventative, and ongoing primary care to patients in the comfort of their homes.

Key Responsibilities

• Provide comprehensive  primary care services in patients’ homes, including routine health maintenance, chronic disease management, preventative care, and acute issue management.
• Conduct physical examinations, order and interpret diagnostic tests, and prescribe medications as appropriate.
• Develop and manage individualized care plans in collaboration with patients, families, and interdisciplinary care teams.
• Coordinate care and referrals with specialists, hospitals, and community resources.
• Educate patients and families on health management, preventive care, and treatment options.
• Document all patient encounters accurately and timely in the electronic health record.
• Travel within the  Chicago area to patient homes, with scheduling supported by our operations team.

Skills, Knowledge, and Expertise

• Active Nurse Practitioner License in Illinois (required).
• Master’s Degree in Nursing (required).
• 2+ years of primary care experience preferred (family practice, internal medicine, or geriatrics).
• Strong clinical assessment and diagnostic skills.
• Excellent communication and interpersonal abilities, with a patient-first mindset.
• Willingness to travel daily to patient homes.
• Experience in home-based care, geriatrics, or palliative care a plus.
